Clinic and doctor booking apps
Doctor appointment app development for clinics, groups and directories
Our doctor appointment app development covers the patient booking app, the doctor and front-desk tools, and the scheduling engine that sits between them and your practice management or EHR system. We build for single clinics, multi-site medical and dental groups, hospital outpatient departments and doctor directory startups in the USA, UK, UAE, Europe and Australia. The Lahore team signs an NDA before discovery and keeps code in your repository from day one.
- Real-time slots from your scheduling system
- Reminders, waitlists and easy rescheduling
- Intake forms before the visit
- Hosting and vendors chosen with HIPAA and GDPR in mind
Who a doctor booking app is for
Three groups ask us for this. Clinics and groups whose phone lines are jammed at opening time and whose no-show rate costs them a doctor’s afternoon each week. Hospital and specialist networks that want one booking front door across locations and specialties. And directory startups that list many independent doctors and want patients to book, not just browse.
Booking is the core here, not the visit itself. If your doctors will see patients over video, the product becomes a virtual care platform, which our telemedicine app development page covers in depth. Many clinics start with booking and add video later on the same backend. Both sit under our healthcare app development work, and a similar scheduling engine powers our salon and spa booking apps, without the health-data rules.
The apps in a doctor appointment system
Patients, doctors and front-desk staff each need their own tools. Most MVPs ship three surfaces on one backend.
Patient app
Search by specialty, doctor, location, language or insurance; see live slots; book, reschedule or cancel; complete intake forms; get reminders; join a waitlist; add family members; view upcoming and past visits, and pay a deposit where your policy requires it.
Doctor app
Today’s schedule, patient details for the next visit, intake answers, availability and leave blocks, running-late notices to waiting patients, and a short note or follow-up request after the visit.
Front desk and admin
Calendar across doctors and rooms, booking rules per visit type, manual bookings from phone calls, check-in, no-show tracking, reminder templates, reports and staff roles with access limited to what each role needs.
Core features per app
The MVP most clinics launch with. Anything else should reduce phone calls, no-shows or front-desk time to earn its place.
| Patient app | Doctor app | Front desk and admin |
|---|---|---|
| Sign-up with phone or email; family profiles; in-app account deletion | Secure login with biometric login and session timeout | Doctors, locations, rooms and visit types |
| Search by specialty, doctor, location and language | Day and week schedule | Booking rules: duration, buffers, new vs returning |
| Live slots and instant booking | Availability, leave and clinic blocks | Calendar view across doctors |
| Intake and consent forms before the visit | Intake answers before each visit | Phone bookings and walk-ins |
| Reminders by push, SMS or email | Running-late notice to waiting patients | Check-in and no-show marking |
| Reschedule, cancel and waitlist | Follow-up booking request | Reminder templates and timing |
| Deposit or cancellation fee, where used | Minimal patient data on the device | Audit log and reports: utilisation, no-shows, lead time |
Scheduling rules and integrations: what drives cost
The scheduling engine
Clinic calendars are full of rules. A new-patient consultation takes 40 minutes, a follow-up 15. Some slots need a specific room or machine. A doctor works Tuesday mornings at one site and Thursday afternoons at another. Some visit types are bookable online and some need triage first. Getting these rules into the engine, and keeping slots correct when the front desk books by phone at the same moment, is most of the backend work. Double booking is the first bug patients and doctors notice.
Practice management and EHR integration
If your clinic already schedules in a practice management or EHR system, that system must stay the source of truth. The app reads available slots and writes bookings through its API or an integration standard the vendor supports. How easy that is varies widely by vendor: some offer open scheduling APIs, others need a partner agreement or a middleware service. We check access before giving a fixed price, because integration is the widest variable in this kind of project.
Reducing no-shows
Reminders at sensible times, one-tap confirm or cancel, and a waitlist that offers freed slots to the next patient automatically do most of the work. Deposits or cancellation fees help some specialties but put off others, so we make them configurable per visit type.
Advanced features worth pricing separately

- Insurance eligibility checks before the visit
- Multi-language and right-to-left interfaces
- Symptom-based routing to the right visit type
- Patient portal with documents and results
- Directory features: reviews, doctor profiles, featured listings

Recommended tech stack for a medical booking app
Our defaults and why. We change them to fit your EHR vendor, hosting rules or in-house team.
| Layer | Default choice | Why |
|---|---|---|
| Patient and doctor apps | Flutter, one codebase | Booking UIs are standard; one team ships iOS and Android |
| Backend | Node.js or Python and Django with PostgreSQL | Slot booking needs transactions and locking to prevent double booking |
| Hosting | A cloud provider that signs a BAA, in the region your rules require | HHS treats a cloud host storing ePHI as a business associate |
| Authentication | Passwordless or passkeys plus biometrics; see authentication and SSO | Fewer password resets for patients, stronger access for staff |
| Admin and front desk | React web app on the same API | Reception works on desktops all day |
| Monitoring | Crash reporting configured to strip health data | Logs must not become an unprotected copy of patient records |
We build the scheduling service as its own API under our mobile app backend development practice, so web booking, the app and your phone team all book against the same slots.
Integrations a doctor booking app needs
- Practice management or EHR: slots, bookings, patient matching and cancellations, through the vendor’s API or a certified middleware service.
- Messaging: SMS, email and push reminders. Each vendor that receives patient data needs the right contract; under HIPAA that means a BAA. Keep reminder text free of diagnosis details.
- Payments: deposits, cancellation fees or self-pay visits through a processor’s SDK so card data never touches your servers. See payment gateway integration.
- Calendars: add-to-calendar for patients, and two-way sync for doctors who keep a personal calendar.
- Maps: clinic finder and directions.
- Video: a managed video SDK if you add virtual visits.
HIPAA, GDPR and store rules for appointment apps
Does HIPAA apply to a booking app?
It depends on the facts. HIPAA applies to covered entities (health plans, clearinghouses, and providers that transmit health information electronically for standard transactions) and to their business associates (HHS Privacy Rule). A US clinic’s own booking app usually handles protected health information, because an appointment with a named specialist reveals something about the patient’s health. A directory startup may or may not be a business associate, depending on whose behalf it acts on. HHS points app developers to the FTC’s tools, and notes that the FTC Health Breach Notification Rule may cover health apps that HIPAA does not.
What it changes in the build
The Security Rule requires administrative, physical and technical safeguards for electronic PHI, such as risk analysis, access control, audit controls and transmission security. HHS says a cloud provider storing ePHI is a business associate even if it holds only encrypted data and no key, so a BAA is required (HHS cloud computing guidance). In practice we pick hosting, SMS, email and analytics vendors that sign BAAs, keep PHI out of push text and crash logs, and log every staff access. Our HIPAA-compliant app development guide goes further.
UK and EU
Identifiable health data is special category data under UK GDPR, so processing needs an Article 6 lawful basis plus an Article 9 condition. Under EU GDPR, breaches involving special category data fall in the higher fine tier.
Store rules
Medical consultations are services, so deposits use a payment processor, not in-app purchase. Apps with account sign-up must offer in-app account deletion, and privacy labels must match what you collect. This is general information, not legal advice; your privacy counsel signs off the data map.
Typical effort estimate (hours) by version
Typical effort estimates (hours), not quotes, for a Flutter patient app and doctor app, a custom scheduling backend, a web front-desk panel and the security work health data needs. Rows are cumulative. The MVP sits just above the 800 to 1,200-hour booking band in our cost guide because of audit logging and access controls.
| Version | What is included | Typical effort estimate (hours) | At the $25–$49/hr market band |
|---|---|---|---|
| MVP (one clinic or group) | Patient app, doctor app, front-desk panel; visit types and booking rules, live slots, reminders, intake forms, waitlist, audit log, BAA-backed hosting | 900 to 1,400 | $22,500 to $68,600 |
| Version 1 (integrated) | Practice management or EHR integration, deposits and cancellation fees, multi-location and multi-language, insurance details, patient documents | 1,600 to 2,500 | $40,000 to $122,500 |
| Scale (network or directory) | Several EHR integrations, directory profiles and reviews, video visits, symptom routing, analytics, external penetration testing, load testing | 3,000 to 4,800+ | $75,000 to $235,200+ |
Dollar figures multiply the hours by the $25–$49/hr band most firms list on the Clutch Pakistan app developer directory. That is market data for illustration, not our pricing. For a booking product that grows into virtual care, read the telemedicine app development cost breakdown; the app development cost guide explains the tiers.
Business models for doctor booking apps
- Clinic-owned: the app is an operating cost that pays back in front-desk time, fewer no-shows and more booked slots. No transaction fees to model.
- Directory subscription: doctors or clinics pay a monthly fee to be listed and bookable.
- Per-booking fee: the directory charges clinics per confirmed appointment. Ask your lawyer whether local rules on patient referral fees affect this model before choosing it.
- SaaS for clinics: license the booking system to many practices, which adds multi-tenancy and per-clinic branding.
- Featured listings: paid placement in directory search, clearly labelled.
How we build it
The same delivery rules as every project; for medical booking we test slot accuracy and data handling hardest.
NDA and discovery
NDA, then one to two weeks of workshops: visit types, booking rules, integration access, a data map of every field and vendor, and who signs which agreement. A fixed price follows.
Design and prototype
Clickable Figma prototypes tested with front-desk staff and a few patients, including older users and accessibility checks.
Two-week sprints
Apps, scheduling service and panel built together, with a TestFlight and Play internal testing build each sprint using test data only.
QA on real devices
Concurrent booking tests, reminder timing across time zones, access-control checks per role and a security review against OWASP MASVS.
Pilot and launch
One clinic or one specialty goes live first, then store submission and rollout across locations.
Support after launch
Monitoring, security patches, OS updates and store requirements under a maintenance plan you choose.
Doctor appointment app development FAQs
How long does doctor appointment app development take?
After one to two weeks of discovery, an MVP with a patient app, doctor app and front-desk panel typically takes 10 to 14 weeks, within our 8 to 16 week MVP range. EHR or practice management integration can extend that, depending on the vendor’s API and approval process. You get a demo build every two weeks.
Is a doctor booking app covered by HIPAA?
Often, but not always. A US provider’s own booking app usually handles protected health information, so HIPAA safeguards and business associate agreements apply. A directory app may fall under HIPAA or under the FTC Health Breach Notification Rule depending on whose behalf it acts. Your privacy counsel makes the call; we build to it.
Can the app use our existing practice management system?
Usually, if the vendor provides a scheduling API or supports a middleware service. The existing system stays the source of truth, and the app reads slots and writes bookings through it. We confirm access and any partner requirements during discovery before giving a fixed price.
How does the app reduce no-shows?
Reminders at set times before the visit, one-tap confirm or cancel, and an automatic waitlist that offers freed slots to the next patient. Some clinics add deposits or cancellation fees for certain visit types. Reports show no-show rates per doctor and visit type so you can adjust the rules.
Can we add video consultations later?
Yes. We design the booking model with a visit type field from the start, so video visits become a new type with a join link rather than a rebuild. Video brings its own vendor agreements and testing, which our telemedicine work covers.
What patient data is stored on the phone?
As little as possible. The app shows upcoming appointments and forms, but sensitive data stays on the server and is fetched over encrypted connections when needed. Doctor apps use biometric login and session timeouts, and push notifications never include diagnosis or visit details.
